Who we are
The Downtown Health Collaborative, or DTHC, is comprised of four partner organizations. Eastern Health provides a variety of services to clients of those organizations at multiple locations. Our partner organizations are:
What we do
We provide complex case management services, primary health-care services, harm reduction programming and intervention for vulnerable sector clients including those who are homeless and precariously housed. We bring health services directly to the community where some of the most vulnerable populations live and seek supportive services. Many of our clients have complex needs.
Who we see
Our focus is on providing services to vulnerable populations who do not have access to services through other means and do not have a primary health-care provider (physician or nurse practitioner).
Our team
We have a range of service providers including physicians, nurse practitioners, registered nurses, licensed practical nurses, social workers, addiction counsellors and occupational therapists.
