Who we are
In a partnership between Eastern Health and the Buckmaster’s Community Centre, we offer nurse practitioner services in a primary health-care clinic setting. The clinic is located in the community centre itself to bring health services directly to the community where some of the most vulnerable populations live.
What we do
We provide primary health-care services as a primary provider, through long-term case management. We also provide care to those who have a private primary physician but may need short-term acute supplemental care.
Who we see
Services at this clinic are open to all members of the surrounding streets and community of the Buckmaster’s Circle area.
Our team
Eastern Health’s services in the clinic are provided through a nurse practitioner. This site also provides public health nursing services.
How to access this service
Services at this site can be made by calling for an appointment. Self-referral is welcome. Appointments are encouraged but walk-in services can be accommodated.
