Who we are
We are a collaborative health practice for patients without a primary health-care provider such as a family doctor or nurse practitioner.
What we do
We operate in a collaborative health model providing the services of multiple providers in one place. A health home model is a hub of health care services that is provided by a variety of health-care professionals that are centred around a patient’s health need(s). At some visits to a clinic, patients may not have to see a physician or a nurse practitioner for their health need(s). The service they need may be provided by another health-care professional instead, such as a social worker, diabetes nurse educator, etc.

The clinic is open to patients who do not have a primary care provider, such as a family doctor or nurse practitioner.
Our team
Services in the clinic are provided through physicians, nurse practitioners, registered nurses and other health-care team members including dietitians, pharmacists, social workers, mental health nurses, diabetes nurse educators, physiotherapists and occupational therapists.
How to access this service
Patients who do not have a primary care provider are connected to Family Care Teams (FCTs) through the Patient Connect NL website or by telephone at 1-833-913-4679. Patient Connect NL will waitlist and connect patients to a FCT closest to where they live. Patients will be contacted by a FCT when the clinic has an available opening to see the new patient. Patients can expect that there may be a waiting period from the time that they register to the time when they are seen at a FCT. We are unable to provide information about the order of placements on the waitlists and when individuals will be contacted for an appointment and to become a patient of the FCT.
What is Patient Connect NL?
Patient Connect NL is a provincial web-based list of individuals who have identified as being without a primary care provider (family doctor or nurse practitioner) in Newfoundland and Labrador. Individuals may register online or by telephone at 1-833-913-4679 to be placed on the list, with the goal of becoming a patient of a FCT and attached to a primary care provider.
