What is the INSPIRED COPD Outreach Program?

Inspired is a program to help manage your COPD at home.

What are the benefits of the INSPIRED program?

  • More confidence in managing your symptoms
  • Less anxiety and stress
  • Fewer emergency department visits and hospital admissions
  • A better quality of life for you

What will kinds of help will I receive from the INSPIRE program?

  • You will get COPD information and support
  • You will get a COPD action plan to help manage COPD flare-ups
  • You will receive suggestions about programs and services that may be helpful
  • You will have the opportunity to consider your goals of care and complete a personal directive document
  • You will receive teaching on how to take your medications and inhalers

How do I participate in the program?

  • You will receive 1-4 visits in your home or at a nearby health care facility by a respiratory therapist and nurse
  • You will also get telephone follow-up and further face to face visits, as needed
  • You will receive a phone number for you to call with questions and concerns

What are the criteria to get into the program?

  • Have moderate to severe Chronic Obstructive Pulmonary Disease (COPD)
  • Have had less than admission to the hospital or ED visit because of your COPD in the past year
  • Interested in self-management
  • Are not a long term care resident
  • Are not currently enrolled in the Remote Patient Monitoring (RPM) Program
  • Live in the St. John’s metro region
